Homemaker Personal Care (HPC) provides personalized, in-home support to individuals who need assistance with daily activities while maintaining independence in their own homes and the community.
At Home SupportAn HPC caregiver visits the individual’s residence on a scheduled basis to provide hands-on assistance with essential daily tasks, ensuring comfort, safety, and well-being. Services may include:
Personal Hygiene & Grooming
Assistance with bathing, dressing, oral care, and other hygiene tasks to promote dignity and self-care.Meal Preparation & Cooking

Your first step is to determine eligibility. If you already have an SSA Case Manager that’s great! Reach out to them to request an assessment and develop a care program so they can match you with the right caregiver for your unique needs and location. If you’re just starting the process you can contact your County Board to request an assessment.

Once you have confirmed eligibility your SSA Case Manger will help you develop an Individual Support Plan (ISP) based on your unique needs and identify which agencies in your area offer services congruent with your plan. You can then review your options and choose your agency. If we’re a match we’d be happy to answer any questions you may have and help you feel confident about your choice!

Once you’ve determined which agency is the right fit, you can communicate your selection to your SSA case manager who will confirm with our agency so we can assign a caregiver based on your ISP, scheduling needs, location and any other preferences you’d like us to factor in to our matching process. Our goal is to help individuals live safely and independently in their homes and communities.

We proudly serve Portage, Stark, and Summit Counties in Northeast Ohio, including the cities of Akron, Cuyahoga Falls, Hudson, Kent, Stow, Streetsboro, Ravenna, and Tallmadge.

Eligibility is determined by the state Department of Developmental Disabilities. If you or your loved one needs support, contact your SSA case manager to discuss eligibility and begin the application process. If you do not currently have a case manager, you can begin the process here.

You have the right to choose your DSP or caregiver. SSA case managers can assist you in finding state funded agencies in your area with services matching your individual needs, preferences, and location. If you would like to request a caregiver from our agency, you can make this preference known to your SSA case manager.

An SSA case manager evaluates each individual’s needs, preferences, and location, then recommends agencies with DSPs who best fits the criteria of your Individual Support Plan (ISP).

Yes, we know how personal this decision is. If you feel a caregiver is not the right fit for any reason, contact your SSA case manager to discuss a reassignment.

If a caregiver is unavailable due to illness or time off, an alternate caregiver will be assigned to ensure continuity of care.
